What do you think about this Poor Design for Roof Plant & Latchway System

Examples of Poor design are quite common and result in many situations presenting unnecessary difficulties for Maintenance. Often just more thought and a little common sense are all that is required. An example of poor design is shown below.
A Latchway System running beneath Plant on a flat roof with no edge protection, an installation without considering those who need to maintain the equipment.
The above is on a Flat roof with no parapet wall. The Latchway System should go round the Plant and equipment to aid safe access and easy maintenance.

It is more than likely that the plant was installed after this UNILINE system was installed. I have come across a lot of examples like this over the years.

I believe that was the case..and the contractor left it as installed the appointed project design coordinator overlooked this important area of works, so i understand. I would have expected the CDM Coordinator to comment prior to construction but he probably was not made aware.
Do any CDM Coordinators / Designers have any tips and advice on this matter.
